Professional Background:
Dr. Lim Chong Soon Daniel is a Senior Family Physician and the Director and Co-Founder of Oaklife Family Clinic.
Dr Daniel graduated from the National University of Singapore (NUS) in 2009. He completed the SingHealth Family Medicine Residency Program and obtained the Master of Medicine (Family Medicine) in 2016. He was elected a collegiate member of the College of Family Physicians Singapore in 2019. He obtained the Graduate Diploma in Family Practice Dermatology from NUS in 2018 and the Graduate Diploma in Mental Health from NUS in 2022.
Prior to setting up Oaklife Family Clinic, Dr Daniel was an Associate Consultant Family Physician and Deputy Director of Punggol Polyclinic. He was Core Faculty with the SingHealth Family Medicine Residency Program from 2017 to 2022, where he taught Family Medicine trainees and was in charge of curriculum planning. He was also Chief Resident for SingHealth Family Medicine from 2017 to 2018. As the Academic Lead of Punggol Polyclinic, he oversaw all training activities in the clinic. He was also Clinical Instructor for the Duke NUS Medical School and Clinical Lecturer for the Lee Kong Chian School of Medicine, where he taught undergraduate students.
Aside from his teaching portfolio, he was in charge of minor surgical procedures in Punggol Polyclinic, where he taught junior doctors how to perform outpatient procedures such as ear syringing, nail avulsions, wound suturing, abscess drainage and steroid joint injections.
As part of his training, Dr Daniel has rotated through most specialty departments, including internal medicine, geriatric medicine, cardiology, dermatology, general surgery, urology, orthopaedics, sports medicine, paediatric medicine, obstetrics and gynaecology (O&G), ear nose throat (ENT), children and adult emergency.
Awards:
In his pursuit for excellence, he has won multiple awards such as the Outstanding Resident Award for all three years of his residency training as well as the Outstanding Faculty Award for three consecutive years from 2018 to 2021. His pleasant demeanour also won him the SingHealth Quality Service Awards - Silver Award in 2016 and Gold Award in 2017.

Dr Jacquelyn Melody

Professional Background:
Dr. Jacquelyn graduated from the National University of Singapore (NUS) with a Bachelor of Science, with merit, in 2007. She took a second degree at the National University of Ireland (Cork) and graduated with a Bachelor of Medicine, Bachelor of Surgery, and Bachelor of Obstetrics in 2012. After returning to Singapore, she joined the SingHealth Family Medicine Residency Program and attained the Master of Medicine (Family Medicine) in 2017. She obtained the Graduate Diploma in Child and Adolescent Health from NUS in August 2023 and was elected a collegiate member of the College of Family Physicians Singapore in November 2023.
Before founding Oaklife Family Clinic, Dr. Jacquelyn served as an Associate Consultant Family Physician with National University Polyclinics (NUP) and Deputy Head of Queenstown Polyclinic. She was also the Lead for the Renal and Urology Specialty Advisory Group in NUP, where she reviewed and updated clinical practice guidelines, training materials, and workflows. Additionally, she was a member of the Paediatric Specialist Advisory Group and the Quality Review Committee in NUP.
While in the institution, Dr. Jacquelyn was involved in teaching undergraduate and postgraduate students. She served as a Physician Faculty for the NUP Family Medicine Residency Program, where she taught postgraduates preparing for the Master of Medicine (Family Medicine) exams. She is also a part-time lecturer at NUS Yong Loo Lin School of Medicine and a Clinical Practice Facilitator at Lee Kong Chian School of Medicine.
During her residency training, Dr. Jacquelyn rotated through various specialties, including paediatrics, children’s emergency, obstetrics and gynaecology, internal medicine, surgery, orthopaedics, emergency medicine, and dermatology. Her extensive experience in primary care, particularly during her years at Queenstown Polyclinic, has honed her skills in holistic patient care. Dr. Jacquelyn is gaining further experience in paediatric care by working weekly at the NUH Children's Urgent Care Clinic.
Awards:
Dr. Jacquelyn’s pleasant personality and commendable service have earned her the Silver Award for the SingHealth Quality Service Awards in 2018 and the Glowing Star Award in NUP in 2022.
